Initial Transformation Consult

Each person's journey begins with a comprehensive, one-on-one 60-minute evaluation designed to fully understand your health, goals, and unique challenges.
NOTE: After this visit, if you decide to continue care, you may choose between the pay-as-you-go option or the transformational membership option.
During this initial session, we will review your medical history, lifestyle, and weight-loss journey, perform vital signs and a focused physical exam, and obtain body measurements along with a detailed body composition analysis. You’ll receive a printed report, and we’ll walk through your results together to help you better understand your body fat, muscle mass, and overall metabolic health.
From there, we develop your personalized plan using our Four Pillar Approach—behavior and mindset, nutrition, physical activity, and medical evaluation. This includes identifying the habits and patterns that impact your progress, building sustainable nutrition and movement strategies, and determining whether medication may be appropriate based on your health history, goals, and hunger patterns.

If medically appropriate, prescription medication may be initiated as early as the initial consultation. Medication eligibility is determined based on factors such as your medical history, BMI, current health conditions, and overall clinical assessment.

Their most recent primary care provider (PCP) progress note

Recent blood work, including a Complete Metabolic Panel (CMP), completed within the past 6 months
If you do not have recent blood work available, that is okay — laboratory testing can be ordered during your initial consultation. If you do not have copies of your recent PCP notes, simply bring the information available from your physician, and we can upload it into your chart as part of your evaluation process.
Following your initial session, you’ll receive a personalized summary and action plan so you know exactly what to focus on moving forward.
Pay-As-You-Go Option Vs. Transformation Membership

Your journey continues with one-on-one 30 minute follow-up sessions (the first at 2 weeks, then at least once monthly) designed to provide ongoing support, accountability, and strategic adjustments as your body, mind, and lifestyle evolve.

Pay-As-You-Go:
You have a two week 30 minute follow-up session then go to once monthly 30 minute follow-up sessions. If you need additional support sessions, you may schedule them as needed. All patients also receive ongoing support features, including provider access through the patient portal between visits.
During these sessions, we review your progress, challenges, and successes, reassess vital signs and physical findings as appropriate, and refine your personalized Four Pillar Treatment Plan based on what is working best and where adjustments may be beneficial.
Transformation Membership:
Includes your first two week follow up session plus your TWO 30-minute sessions per month. This includes enhanced support features, including quicker portal messaging access to your provider between visits.
During these sessions, we review your progress, challenges, and successes, reassess vital signs and physical findings as appropriate, and refine your personalized Four Pillar Treatment Plan based on what is working best and where adjustments may be beneficial.
Body composition analysis is repeated (for both options) to track fat loss, muscle preservation, and metabolic progress, and medications are adjusted when indicated. Each session is focused, personalized, and designed to keep you moving forward with clarity and confidence.
​​​Regardless of which option you choose, after your follow up sessions, you’ll receive a personalized summary and action plan by email so you know exactly what to focus on moving forward.

Quarterly Metabolic Review
Your journey and progress is celebrated at each session, but with much more detail at your quarterly metabolic review.
This extended, one-on-one 60-minute session is designed to highlight your wins, review meaningful progress, and reinforce the many ways your health and life are transforming—helping to build motivation and sustain long-term success.
This session focuses on both objective and subjective progress.

Objective:
We review detailed body composition changes compared to 90 days prior, including total weight loss (in pounds and percentage), visceral fat reduction, muscle mass preservation, and hydration levels. We also track body measurements, blood pressure trends, changes in clothing fit, and, if desired, progress photos to visually capture transformation, among other metrics.
Subjective:

We assess how you’re feeling overall—your energy, confidence, mood, stress levels, anxiety, and pain—ensuring that your progress reflects meaningful improvements in your day-to-day life.
This important session ensures your progress goes far beyond the scale—highlighting the many meaningful wins that deserve to be recognized and celebrated. This is what true metabolic health looks like, where lasting change is built, and where real transformation happens.
